EMERGENCY MEDICAL TECHNICIAN (EMT) INITIAL APPLICATION

For INITIAL Certification Only

You must have documents electronically copied (cards & forms)
PRIOR to starting the online application process.

Be sure to read carefully and answer all questions.

You will need the following:

1. Copy of a current driver’s license or a government issued identification card

2. Copy of current National Registry Certification (NREMT card AND certificate)

3. Copy (front and back) of current CPR Certification taught to the curriculum standards of the American Heart Association, American Red Cross or the National Safety Council at the Health Care Provider or equivalent level

4. Copy of your EMT training program course completion certificate, must include epi-autoinjector, naloxone and glucometer certifications

5. A live scan criminal background check, completed no more than 60 days prior to the date the certification application is submitted. The El Dorado County EMS Agency Live Scan form must be used, and a copy of the completed form (once the live scan has been performed and signed by the technician) must be provided to the El Dorado County EMS Agency. https://www.edcgov.us/government/ems/forms/documents/LifeScanApplication012914.pdf

Background Disclosure

The Medical Director shall deny or revoke an EMT certificate if any of the following apply to the applicant:

  1. has committed any sexually related offense specified under Section 290 of the Penal Code
  2. been convicted of murder, attempted murder, or murder for hire
  3. been convicted of two or more felonies
  4. is on parole or probation for any felony
  5. been convicted and released from incarceration during the preceding fifteen years of the crime of manslaughter or involuntary manslaughter
  6. been convicted and released from incarceration during the preceding ten years for any offense punishable as a felony
  7. been convicted of two misdemeanors within the preceding five years for any offense relating to the use, sale, possession, or transportation of narcotics or addictive or dangerous drugs
  8. been convicted of two misdemeanors within the preceding five years for any offense relating to force, violence, threat or intimidation
  9. been convicted within the preceding five years of any theft related misdemeanor has committed any act within the preceding seven years involving fraud or intentional dishonesty for personal gain

Maintaining Certifications

I understand it is my responsibility to have current/valid certifications on file at all times with the El Dorado County EMS Agency. This means if a certification expires during my EMT certification period, it is my responsibility to submit a current copy to El Dorado County EMS Agency. (Emailing a copy is the preferred method of submission)

BLS Provider CPR Certification:

CPR certification must be taught to the curriculum standards of the 2015 American Heart Association (AHA) Basic Life Support (BLS) for Healthcare Providers CPR/Automated External Defibrillator (AED) Program, or equivalent level.  Online CPR courses are not accepted.

Fingerprinting/Live Scan:

EMT School Course Completion Certificate:

Per California Code of Regulations, Title 22, Division 9, Chapter 2. § 100077.

  • An approved EMT School training program provider shall issue a tamper resistant course completion record for each person who has successfully completed the EMT School training program. 
     
  • The EMT School training program shall consist of not less than one hundred seventy (170) hours.
     
  • The EMT School training program course completion record is valid to apply for EMT Certification for a maximum of two (2) years from the course completion date.

Application Fees

A non-refundable application fee must be paid in order to process an application. The fee for an Initial EMT Application is $100.
